Dose escalation as a strategy to improve antibiotic efficacy. Dose escalation either
by administering higher doses or administering a dose more frequently achieves only
small increments in efficacy because there is little increase in the time during which
the drug concentration is above the minimum inhibitory concentration (MIC).
